Privacy policy
Effective date: September 2026
This policy covers Leland Care’s website and advertising.
1. Scope of this policy
Leland Care (“Leland,” “we,” “us,” “our”) provides nurse advocate care coordination services through its affiliated medical practices, Leland Medical Group, P.A. (Florida) and Leland Medical of New York, P.C. Leland Care, Inc. manages operations for these practices. This Privacy Policy describes how we collect, use, disclose, and protect personal information gathered through our website at lelandcare.com, and through our digital advertising, including lead forms on Facebook, Instagram, Google, and similar platforms (together, our “Services”).
This policy applies to prospective patients, family members and caregivers inquiring on a patient’s behalf, referring providers, and other visitors to our Services. It does not apply to information we collect once you become a Leland Care patient in the course of treatment. That information is protected health information (“PHI”) governed by the Health Insurance Portability and Accountability Act (“HIPAA”) and described in our separate Notice of Privacy Practices, available upon request and provided at enrollment.

2. Information we collect
Information you provide directly
- Name, phone number, email address, and mailing address, when you contact us, request information, or submit a form
- Information about a prospective patient’s health status or care needs that you choose to share with us before they become a patient (for example, Medicare status, diagnoses, or living situation), so that we can assess eligibility and fit
- Any other information you share with us by phone, email, text, or chat
Information collected automatically
- Device and usage data: IP address, browser and device type, pages visited, referring website, approximate location, and timestamps, collected through server logs and analytics tools
- Advertising identifiers and interaction data collected through cookies, pixels, and tags (for example, the Meta Pixel and Google Ads tag), described further in Section 6
Information from third parties
- If you submit a lead form through Facebook, Instagram, Google, or a similar platform, that platform shares the information you entered with us under its lead-ads terms; we treat it the same as information you give us directly
- Aggregated or de-identified campaign performance data from advertising platforms, which does not identify you individually
3. How we use your information
- To respond to your inquiry and follow up by phone, text, or email about our services
- To assess eligibility for care and connect you with a dedicated nurse advocate
- To send appointment reminders, follow-ups, and family updates you request
- To operate, secure, and improve our website
- To measure and improve our advertising, including which campaigns and pages lead to genuine inquiries, and to avoid showing ads to people who have already contacted us
- To comply with applicable law, respond to legal process, and enforce our agreements
We do not sell your personal information for money, and we do not use your information for purposes unrelated to those described here without your consent.
4. Consent to be contacted
By submitting a form on our Services, you authorize Leland Care to contact you at the phone number and email address you provided, by call, text message, and email, regarding your inquiry, including through an automatic telephone dialing system or prerecorded message where permitted by the Telephone Consumer Protection Act and applicable state law. Message and data rates may apply, and message frequency varies. This consent is not a condition of receiving care from Leland Care. To revoke consent, see Section 8.
5. Legal basis for processing
Where required by applicable law, we process personal information on the following bases: your consent (for marketing contact and advertising cookies); our legitimate interests in operating and promoting our practice, weighed against your privacy interests; and compliance with our legal obligations as a health care provider. You may withdraw consent at any time as described in Section 8, without affecting the lawfulness of processing before withdrawal.
6. Cookies, pixels, and online tracking
Our website uses cookies and similar technologies, including the Meta Pixel and Google Ads conversion tag, for the following purposes:
- Essential: operating core site features and security
- Analytics: understanding how visitors use our site so we can improve it
- Advertising: measuring which ads and pages lead to inquiries, and showing our ads to people likely to be interested (interest-based or “retargeting” advertising)
You can control cookies through your browser settings, and limit interest-based advertising through your device settings or industry tools such as the Digital Advertising Alliance’s opt-out page. Our website does not currently respond to browser “Do Not Track” signals, but does honor recognized opt-out preference signals (such as Global Privacy Control) where required by law, as described in Section 8.
7. How we share your information
We do not sell personal information for money. We disclose personal information only as follows:
- Service providers: vendors who perform services on our behalf, such as scheduling, telephony, messaging, hosting, and analytics, under contracts that limit their use of your information to our instructions
- Advertising platforms: Meta, Google, and similar platforms that help us reach and measure prospective patients. Because these platforms may use shared data for their own advertising purposes, several state privacy laws classify this activity as a “sale” or “share” of personal information even though no money changes hands. Section 8 explains how to opt out
- Your care team: once you or your loved one becomes a patient and authorizes coordination of care
- Legal and safety purposes: to comply with a subpoena, court order, or other legal obligation, or to protect the rights, property, or safety of Leland Care, our patients, or the public
- Business transfers: in connection with a merger, acquisition, financing, or sale of assets, subject to confidentiality obligations
8. Your privacy rights
You may exercise the following rights by contacting us at hello@lelandcare.com or (914) 530-0242. We will verify your request and respond within the time required by applicable law (generally 45 days, extendable as permitted).
- Access: request a copy of the personal information we hold about you
- Correction: request that we correct inaccurate information
- Deletion: request that we delete your personal information, subject to legal and operational exceptions (for example, records we must keep for regulatory compliance)
- Opt out of sale or sharing: direct us to stop sharing your information with advertising platforms for cross-context behavioral advertising, as described in Sections 6 and 7
- Opt out of specific channels: reply STOP to any text message, use the unsubscribe link in any email, or tell us by phone to stop calling
- Appeal: if we decline a request, you may appeal by replying to our decision letter; we will respond to appeals within 45 days
Residents of California, Colorado, Connecticut, Virginia, Utah, and other states with comprehensive privacy laws have the rights described above under those laws, including the right to non-discrimination for exercising them, as described in Section 10. We will not sell or share, and do not knowingly sell or share, the personal information of consumers we know to be under 16 years old.
9. Health information and HIPAA
Information you or a family member share with us before becoming a patient (for example, through a lead form, phone inquiry, or intake conversation) is collected as part of our marketing and intake process and is governed by this Privacy Policy, not by HIPAA, because a treatment relationship does not yet exist.
Once you or your loved one enrolls as a Leland Care patient, information created or received in the course of care becomes protected health information governed by HIPAA and our Notice of Privacy Practices, which we provide separately at enrollment and describes your rights regarding PHI, including access, amendment, and accounting of disclosures. We do not use advertising pixels or share data with advertising platforms using information collected after a treatment relationship begins.
10. Nondiscrimination
We will not deny you services, charge different rates, or provide a different level of care because you exercised any privacy right described in this policy.
11. Data security
We maintain administrative, technical, and physical safeguards designed to protect personal information against unauthorized access, disclosure, alteration, and destruction, including encryption in transit, access controls, and vendor due diligence. No method of transmission or storage is completely secure; we cannot guarantee absolute security.
12. Data retention
We retain personal information only as long as reasonably necessary for the purposes described in this policy, or as required by applicable law and recordkeeping obligations for health care providers, after which we delete or de-identify it.
13. Children’s privacy
Our Services are directed to adults and are not intended for children under 13. We do not knowingly collect personal information from, or direct advertising to, children under 13, consistent with the Children’s Online Privacy Protection Act. If we learn we have collected such information, we will delete it.
14. Third-party links
Our Services may link to third-party websites, including advertising platforms and payer or Medicare resources. We are not responsible for the privacy practices of those third parties, and encourage you to review their policies.
15. Changes to this policy
We may update this policy periodically to reflect changes in our practices or applicable law. The current version is always posted at lelandcare.com/privacy with a revised effective date. Material changes will be highlighted at the top of this page.
16. Filing a complaint
If you believe we have not handled your personal information in accordance with this policy or applicable law, contact us first at hello@lelandcare.com so we can address your concern.
